Key Responsibilities
The Physician Services Population Health Manager will be responsible for developing and implementing strategies to enhance care coordination, manage chronic diseases, and improve overall community health. This role requires a strategic thinker with a strong background in success in value-based care arrangement and other advanced payment models, comprehensive knowledge on how to lead a successful clinically integrated network, strong comprehension with payor contracts.
Required Qualifications
Minimum 5 years experience in healthcare management, with a focus on geriatric population health, care coordination, or chronic disease management Experience with HCC/ICD10 coding and a strong understanding of its impact on healthcare reimbursement and quality metrics Proven track record of developing and implementing successful population health programs Understand levels of care defined through the Medicare benefit (Certified Home Health, Hospice, etc.) Strong leadership and team management skills Excellent analytical and problem-solving abilities Effective communication and interpersonal skills Ability to build and maintain relationships with diverse stakeholders Proficiency in data analysis and the use of population health management software Reliable transportation Desired
Qualifications
