What You'Ll Do

Direct Clinical Services Conduct comprehensive biopsychosocial assessments to evaluate client needs, strengths, and risk factors Provide individual, family, and group therapy using evidence-based modalities (CBT, DBT, trauma-informed care, motivational interviewing, etc.) Develop and implement individualized treatment plans in collaboration with clients and care team members Monitor and document client progress, adjusting treatment plans as clinically indicated Provide crisis intervention, safety planning, and stabilization services as needed Care Coordination & Advocacy Collaborate with physicians, nurses, case managers, and community health workers to ensure integrated, whole-person care Connect clients with community resources including housing assistance, food security programs, transportation, legal aid, and

What We’Re Looking For

Active Licensed Clinical Social Worker (LCSW) license or Licensed Professional Counselor (LPC/LMHC) 3+ years of post-licensure clinical experience (community mental health, integrated care, or similar settings preferred) Comfort providing therapy independently within a supported care model Strong clinical judgment in assessment, diagnosis, and treatment planning Clear, professional communication and collaboration with care teams Comfort using telehealth platforms and clinical documentation tools Commitment to full-time work and continuity of care
